For the sole consideration of acknowledging this document and participating in the Mile High Lacrosse League, Player as signed, the receipt and sufficiency of which are hereby acknowledged by these presents do for myself, my heirs, personal representative, subrogees and assigns, hereby release, acquit and forever discharge the Mile High Lacrosse League, its officers, directors, and participants and the referees of said league, and the heirs, personal representatives, of and from any and all action, claims and demands, damages, costs, loss of services, expenses and compensation on account of or in any way growing out of any known and unknown injuries or damages resulting or to result from the Players participation in the Mile High Lacrosse League for any reason of negligence or conduct of the league. The player acknowledges that the participation in the league is designed for fun and active participation and as such releases, indemnify and saves harmless the people referenced above from all claims and demands, costs, loss of service, expenses and compensation on account of or in any way growing out of the players participation in the league for the current upcomming season.
All players registering to play in the Mile High Lacrosse League must have celebrated their 18th birthday. If there is any doubt surrounding your age, identification may be requested to support your legal age. The valid forms of identification are federal, state or local photo I.D. card. If the player does not immediately have access to identification, they must provide it by the start of their next scheduled game. If they are unable to present the I.D. as requested they shall be subject to suspension from the league until said identification can be provided and age can be verified.
Fighting and foul language are expressly prohibited in this league and will be dealt with by appropriate sanctions as determined by referees during the game and the league board subsequent to the game in which the event occurred. Sanctions may range from penalties of single game suspensions to complete ejection from the league and are at the discretion of the board. By signing this waiver, the payer expressly agrees to these terms and conditions of play and understands that there are no rights to appeal the board decisions on these matters and no rights to a refund of any dues or fees payed to the league if the player is disqualified from any game or the remainder of the season as a result of his conduct.
Player hereby affirms that this Release is in every sense total and complete and covers not only those claims that are known, but such claims as may in the future arise on account of any loss, detriment or damage of any kind or character whatsoever sustained by Player as a result of the participation described above. Player for a portion of the consideration herein set forth, expressly undertakes and assumes the risk that is inherent in the participation in a contact sport such as lacrosse and the player is fully informed as to the risks inherent in the participation in the game of lacrosse. Player further expressly understands and agrees that the signing of this Waiver and Release of All Claims shall be forever binding and no rescission, modification or release of the Player by the league from the terms of this Agreement and Release will be made for any mistakes as set forth herein.
